Who we are

This website is operated by Colleen Makowsky, MA, LPC, a licensed professional counselor in New Jersey (NJ LPC #37PC00901900), providing telehealth therapy to adults and couples across New Jersey. The website address is colleenmtherapy.com.

What this policy covers

This policy explains how information is handled on this website. It does not cover clinical records. Information collected once you become a client, including session notes and treatment records, is handled under a separate Notice of Privacy Practices provided before your first session.

Information you provide

The consultation button opens an intake form hosted by Google Forms, which asks for your name and how you would prefer to be contacted. Please do not use it to describe symptoms, diagnoses, or other clinical detail — there is time for that on the call.

If you email colleenmakowsky@audramiranda.net or call and leave a voicemail at (917) 519-8447, that message is kept so it can be answered.

Information collected automatically

The web host records standard server logs: IP address, browser type, pages requested, and the date and time. These are used for security and site reliability.

This site does not use analytics, advertising, or social media tracking, and there are no third-party trackers or pixels. Cookies are set only for logged-in administrators; visitors browsing the site are not given tracking cookies.

Fonts are loaded from Google Fonts, which means Google receives your IP address when a page loads.

How information is used

To respond to consultation requests, to schedule and provide therapy services, to keep the site secure and functioning, and to meet legal and professional record-keeping obligations. Information is not sold, rented, or traded, and is not used for marketing.

Who has access

Colleen Makowsky. Information also passes through the service providers that operate the site and practice, namely the web host and Google, which hosts the intake form. Information may be disclosed where required by law or professional obligation, such as a court order or a legally mandated report.

How long information is kept

Consultation requests from people who do not become clients are kept only as long as needed to respond, then deleted. Client records are kept for the period required by New Jersey law and professional standards. Server logs are retained on a short rolling basis by the host.

Your choices

You can ask what information is held about you, ask for corrections, or ask for deletion, except where it must be retained for legal, professional, or record-keeping reasons. Requests go to colleenmakowsky@audramiranda.net.

Security

Reasonable measures are used to protect information, including serving the site over HTTPS. No website or email system is completely secure, and no guarantee is made.

Not for emergencies

This website, the intake form, email, and voicemail are not monitored continuously and are not for emergencies. If you or someone you know is in immediate danger, call 911, or call or text 988 for the Suicide and Crisis Lifeline.

Children

This site is intended for adults. Services are provided to adults and couples, and information is not knowingly collected from anyone under 18.

Changes

If this policy changes, the revised version will be posted here with a new last-updated date.
