Colleen Makowsky accepts most major New Jersey insurance plans — Aetna, Cigna/Evernorth, Optum/UnitedHealthcare/Oxford, Horizon Blue Cross Blue Shield of New Jersey, Oscar, Medicare, and most employee assistance programs. Most clients pay only a copay. For Medicaid clients, Colleen is happy to refer you to a colleague who works with that population — Medicaid is not accepted. If you have out-of-network benefits, a superbill is available on request.

Medicaid is not accepted by my practice. If you have Medicaid and need counseling in New Jersey, I can refer you to a colleague who works with that population. Reach out — we’ll talk through it on the free 15-minute call, and I’ll point you somewhere good.
This is a sitewide commitment to being clear. I’d rather say no upfront and refer well than leave you uncertain.
What you’ll actually pay
Rates are set by your carrier benefits — not by a flat session fee. Most clients with in-state behavioral-health coverage pay only a copay, which is usually somewhere between $0 and $40 depending on the plan and the deductible status.
If your plan has a deductible that hasn’t been met, you may pay the full session rate (carrier-contracted) until the deductible is hit, then a copay or coinsurance after that.
For out-of-network clients, I provide a superbill on request. Your OON benefits determine the reimbursement rate — typically 50–80% of the session rate, depending on your plan. The superbill is what you submit to your insurance for reimbursement; you pay me directly, and they pay you back.

What a session looks like, week to week
Standard for both individual and couples work. Range reflects how sessions actually unfold — some are tighter, some run longer.
NJ statewide telehealth. No physical site, no public street address.
Varies by client. Some work shorter (a few months); some longer. We check in regularly about whether what we’re doing is still useful.
Frequency can shift to bi-weekly or monthly after the initial weekly cadence — depends on the work and what’s serving you.

Does Cigna or Evernorth cover therapy in New Jersey?
Yes. Evernorth is Cigna’s behavioral-health arm — most Cigna NJ plans route through it for therapy benefits. Standard plan structure applies (copay or deductible + coinsurance). We verify benefits before the first session.
Does Horizon BCBS NJ cover therapy with you?
Yes — Horizon Blue Cross Blue Shield of New Jersey is accepted. Behavioral health benefits apply per your plan. NJ Horizon plans vary widely on copay structure and deductible — we’ll verify before the first session.
Does Optum / UnitedHealthcare / Oxford cover therapy with you?
Yes — all three brands share Optum’s behavioral-health network, and that’s the one I’m in. Benefits work the same across the UHC and Oxford NJ plans. Copay structures vary by employer plan, so we verify before the first session.
Does Medicare cover therapy in New Jersey with you?
Yes — Medicare is accepted. The outpatient mental-health benefit applies to my sessions. You’d pay the standard Medicare copay structure for outpatient behavioral health. We verify benefits before the first session.
What does it cost if I have out-of-network insurance?
For out-of-network clients, I provide a superbill on request — that’s the document you submit to your insurance to claim reimbursement. Your OON benefits determine the rate, typically 50–80% of the session rate. You pay me directly at the time of session, and your insurer reimburses you. Do you offer sliding scale?
